We have created this tool to help verify the identities of our staff, when liaising with outside medical colleagues, by phone.
On occasion, we will contact hospitals an medical facilities, by phone, to follow-up on the well-being of our patients.
If a hospital or medical facility requires us to prove our identity, they will be directed to this page and asked to provide a name and four-digit code. Our staff will then be able to verify their own identity by repeating the specified four-digit code back to that particular hospital or medical facility.
